The director stated that the operation was out of ratio on 5-11-26.

The director did not demonstrate good judgement when the director did not notify the child's parent that their child had been removed from the daycare bus for safety reasons and needed to be picked up from the school.

It was determined that a caregiver physically disciplined a child as a form punishment. (Note: The caregiver is no longer employed at the operation).

There are 5 children in the infant classroom whose feeding instructions have not bee signed or updated by the parents in the last 30 days.

Two children were missing two vaccinations from their immunization records that I reviewed during the inspection.

Documentation for the emergency drills was unavailable for review for the months od April-July during my inspection. This was corrected at the inspection when the director back tracked and documented the dates, times,a nd minutes it took for the children to evacuation on the dates that were missing.

The Pre-Employment Affidavit for Applicants for Employment at Certain Child Care Operations form was missing from 10 staff member's files.

There is sufficient evidence to determine that the teacher thumped the child on the face as a form of discipline.

There is sufficient evidence to determine that the teacher displayed a lack of self-control and good judgement on two occasions when interacting with children at the operation.

There is sufficient evidence to determine that staff was aware that the child's arm was hurting while in care and did not contact the parent until pick up.

The operation's last gas leak test was completed on 7/21/2022. The gas leak inspection is scheduled for 9/30/2024.

It was determined that a caregiver has spoken to children using non-positive statements.

Two caregivers who work with children under the age of 24 months, lack current traiing in SIDS/Shaken Baby/Early Brain Development.

Feeding instructions for 2 infant children, had not been updated within the past 30 days. (Both infants are not yet on table food).

Infant children are not getting outdoor time.

The operation failed to timely report a situation that put a child at risk.
